A health maintenance organization will provide you with a local network of hospitals, doctors, and health care professionals that you will have to choose from. You will have to choose a primary care provider that is in your network for your medical care.
An exclusive provider organization offers networks of participating providers from which to choose. Most of the time these will not provide coverage for any out-of-network care, which means if you go to a doctor that is not in the network you will have to pay the full amount.
The point of service plans will combine the features of the HMO and a PPO. The network is often smaller and the costs for care in the network are often lower. You have to choose a primary care provider within the network.
Preferred provider organizations offer a large network of providers so you have more that you can choose from. You are not required to choose a primary care physician.
